Just out of curiosity, how often does this happen to you?
I frequently get double-cupped when I order a frozen/cold drink, such as a Carmel Frap. I don't know why this happens. Perhaps it's intentional so my poor little hands won't get too cold? Perhaps it's just a case that the cups stick together and the barrista doesn't realize it when they grab the cup(s) to prepare my drinks? Either way, it's quite wasteful. When I stop to consider just how many Starbucks there are around the globe and how many people per day could possibly be getting double-cupped, it seems exceedingly wasteful and I am astonished that this could possibly be in practice (either intentionally or unintentionally) globally.
Starbucks, PLEASE STOP THIS MADNESS NOW! One cup will be more than sufficient to contain my drink, thank you. :-) I don't need two. Save the second cup for the next person. You may save yourselves (and, as a result, your customers) some money and you'll be doing your part to help preserve our environment as well.

Firstly. The reason you will often get a double cupped cold drink is because in the process of making it, you end up with beverage dripping down the side of your cup. And you might wonder why this matters. Grab a towel or a napkin and wipe it off right? Wrong. Baristas are given three minutes (read THREE minutes) from the time you place your order until you get your drink. EVERY second counts. It's easier to drop your cup into a second cup to save you from getting your hands dirty and save on seconds than it is to stop, wipe down the cup and make sure the customer (not you in particular) doesn't bitch and moan about being handed a 'dirty cup.'
So.. there's that. My devils advocate bit is this...
If you are indeed so concerned about the environment, stop using Starbucks cups all-together!! Invest in a travel mug that we sell at the store, one that can hold both hot and cold drinks and bring that with you whenever you go. Not only does it save you 10cents on every drink you buy, you'll also be saving yourself and the environment not ONE, but obviously sometimes TWO cups ;)) Just a thought.
